# How to Access and Edit Patient Records
## Accessing and editing patient records from the EMR
Click the "Enter patient name, On the global search bar at the top of the screen, enter the patient name, phone number, or date of birth to search for the patient.
You can also go to a patient's chart by clicking on the patient name anywhere on the EMR.
On the top of the chart, to the left of the patient name, you will see the patient's profile picture, where you can add, delete and replace the image.
Below the patient name there are 8 icons, each of which represent a separate option.
The first icon will tell you if the patient has any valid ID's uploaded.
The second icon is for insurance cards, which lets you know whether the patient has any valid insurance cards uploaded.
The third icon is for printing the patient face sheet.
The fourth icon will show you all the upcoming appointments created for the patient.
The fifth icon will show you the to-dos created for the patient.
The sixth icon is for stickies, clicking on which will open the stickies side panel for you to view.
The seventh icon is for collecting deposit from the patients.
The eighth icon is to adjust patient payments.
Now let's look at all the tabs you will find under patient charts.
The first one is Demographics which displays all relevant personal information of the patient. Editing any information here which will be reflected throughout the system.
Once you make any changes, click "Update" on the bottom of the screen to save them.
The next tab is "PCC", which contains all the details of the patient's agreed upon communications details.
The next tab is "Visits", which contains details of all the Upcoming and Past visits. Providers can also view visit notes from past visits and add addendums to them from here.
You can also view the finalized visit note PDFs from here as well.
The third tab is Ledger, which contains the transaction summaries of all encounters for that patient as well as the transaction log for each transactions.
You can select the encounters based on the dates to view the individual transaction logs.
You can also print patient statements by clicking on the print icon on top of the transaction log.
The next tab on patient charts is Insurance, which includes the patient's insurance details. You can add new insurance from here by clicking on the "+" beside "Add Insurance Policy". You can edit information and delete them from here as well.
You can also upload insurance cards here by clicking on the "Upload" button beside "Add Insurance Cards".
Then comes the "Meds & Allergies" section which includes details of any medicines the patient may be taking and any allergic reactions they may have to certain medicines.
Next comes the "Vitals" tab, which shows the patient’s recorded vital signs and their trends over time, including blood pressure, heart rate, respiratory rate, temperature, and oxygen saturation.
Next is Patient Histories, which contains the details of the patient's social, surgical, medical, family histories, and any non-drug allergies the patient may have (e.g. nut allergies, dust allergies, etc.)
The next tab is for Review of Systems which includes any other bodily condition the patient may have that the doctor should be aware for proper diagnosis.
The last one is the Multimedia tab where you can find any multimedia that the patient has uploaded in the intake forms (e.g. X-Ray images, PDFs, etc.).
You can also upload multimedia here by clicking on the "Upload" button.
