[Populate Continuous Care (PCC)](https://knowledge.populateemr.com/populate-continuous-care-pcc?hsLang=en) October 18, 2026 # How to Send PCC Consent Forms to Patients for Enrollment? ## This guide details how to resend consent forms from a patient's visit note, the PCC dashboard, or the patient's chart; track the patient's signing experience, and complete final program enrollment afterwards. ### How to Send PCC Consent Forms to Patients for Enrollment Activating Populate Continuous Care (PCC) for your organization does not automatically enroll patients. Each patient must sign a PCC consent form before they can be enrolled in care management programs such as TEMPO, RTM, or CCM. **Consent forms are sent automatically upon enrollment, but they can also be resent at any time from multiple areas within the EMR.** This guide details how to resend consent forms from a patient's visit note, the PCC dashboard, or the patient's chart; track the patient's signing experience, and complete final program enrollment. > **Prerequisite:** PCC must be enabled for your organization before proceeding. #### Resend the Consent Form from a Visit Note The consent form can be resent directly from the Visit Note's PCC Panel. Until the patient signs, the panel shows Consent Pending with the message Program reminders start once signed. Click Resend to send the consent form to the patient if they have not signed it yet. ![](https://usercontent.us.prod.clueso.io/0ee74cae-77f1-4ef9-a322-2a7d9ec1a279/4418d309-2a72-45c9-8632-66d5fa2119b0/11772522-4b50-4b52-8fb9-2c4191c2c934/images/84bdfce0-2aab-4790-81bd-127fb235e805.png) **\> Tip:** The panel also shows each program's eligibility and the reason when a program isn't available (for example, No active insurance or No qualifying diagnosis on visit notes). #### From the PCC Dashboard Alternatively, you can navigate to the **PCC Dashboard** and resend consent forms to eligible patients from the **Overview** tab by clicking the **Send Consent Form** button located on the far right of the page, next to Enroll.   ***![](https://knowledge.populateemr.com/hubfs/image-png-Oct-17-2026-11-50-12-9051-PM.png)***  #### From the Patient Chart (PCC Tab) Navigate to the **Patient's Chart** and go to the **PCC** tab. Under **Communications**, you will see the **Send Consent Form** button available. To do this, go to the visit note, and click the PCC button in the header. The panel now shows **Ready to enroll** and lists the programs this patient is eligible for. Select the programs you want and click **Enroll in programs** (the button shows how many you selected, for example Enroll in 3 programs). For CCM and RTM, eligibility depends on the diagnosis codes in the note. **Podiatry practices qualify for TEMPO and RTM.
