# Costs and Economics Associated with the Programs
## Overview of PCC pricing, reimbursement rules, and potential patient out-of-pocket cost
**What does PCC cost?**
There are no upfront costs associated with PCC. The practice pays Populate only when an insurance company actually reimburses a PCC service.

The 40% Populate fee applies only to insurance payments actually collected. If an insurer partially reimburses a claim, the 60/40 split applies to the amount actually received. If the claim is denied or the insurer pays nothing, the practice owes Populate nothing for that claim.
Populate receives none of the patient's payment. The practice keeps 100% of deductibles, copays, coinsurance, and other patient collections.
**Can patients have out-of-pocket costs?**
**Yes.** PCC services are insurance-billed medical services and normal patient cost-sharing can apply to all three PCC programs. For traditional Medicare patients, this commonly includes Part B deductible and coinsurance obligations. For CCM in particular, practices should expect some patients to receive recurring monthly patient responsibility, which can generate questions or lead some patients to opt out. Supplemental insurance may cover some or all of the patient's responsibility.
**Is reimbursement guaranteed?**
**No.** Coverage and reimbursement depend on the patient's insurance, eligibility, medical necessity, documentation, and payer requirements. Populate does not perform benefits verification and does not guarantee payment.
**If insurance does not reimburse the service, Populate does not receive a fee.**
