What PCC is?
Populate Continuous Care helps your practice stay connected to patients between visits. PCC collects information about how patients are doing, identifies patients who may warrant intervention, recall, or care-plan adjustments, and supports the documentation and billing associated with reimbursable continuous-care services.
Patients can be enrolled in one, two, or all PCC programs available to their specialty.
PCC currently includes 3 programs:

Podiatrists cannot enroll patients in CCM. PCC is activated at the organization level, but patients are never automatically enrolled. A provider chooses which eligible PCC program or programs are appropriate for each patient. Each enrollment lasts one year.
