What You Are Agreeing to upon PCC Activation?

Activating PCC makes all PCC programs supported for your specialty available across your organization. Activation is completed by an authorized provider through an electronic signature. Populate records the provider, date, and version
of the terms accepted.

By activating PCC, your practice acknowledges that:

1. Your practice decides which eligible patients to enroll.
2. Your practice remains responsible for clinical decisions, medical necessity, documentation review, coding, and compliance with applicable payer requirements.
3. Populate does not perform benefits verification or guarantee reimbursement.
4. PCC patient communications come from dedicated Populate phone numbers rather than your practice's existing phone number.
5. PCC program names, communication cadence, reminder timing, and phone numbers are standardized and are not configurable.
6. Organization-level clinical alert thresholds are configurable.
7. Populate operates as a technology and service provider on behalf of your practice under applicable HIPAA and BAA requirements.

There is no upfront cost to activate PCC and no charge simply for enrolling a patient.
